F R E S H M E N
Step 1 — Admission Test & Interview
  1. Fill out the Application for Admission form. Forms are available at the Admissions & Registration Office
  2. Pay the examination fee at the Cashier who will issue an official receipt (OR) for the amount paid.
  3. Present filled out Application for Admission together with the required documents, to the Student Affairs Office (SAO)
  4. SAO will issue the stub indicating the date when admission examinations shall be taken
  5. Take the admission examinations and show up for the interview on the date written on the stub
  6. The admission examinations result will be sent to the email address stated in the application form.
    Please note that all courses have limited slots, and when all slots are filled up, new applicants will be automatically included on the waitlist.
  7. If qualified for admission, students are advised to seek medical certification that they are fit to study from a CLDHEI-accredited physician/clinic before the date of enrollment.
  8. Requirements:
    • 2 pcs. 2" x 2" colored ID picture
    • Grade 11 rating
    • Enrollment certificate from adviser or school head
    • Official receipt (OR) of the payment for the examination fee
Step 2 — Registration & Assessment
  1. Present requirements to the Admissions & Registration Office at any day during the scheduled enrolment period
  2. Fill out the Student Personal Record card. The card will be provided by the Admissions & Registration Office
  3. Read and sign the Data Privacy Notice/Consent form
  4. Submit the two forms to the Admissions & Registration Office
  5. The Admissions & Registration Office will issue a copy of your Pre-Assessment form. The Pre-Assessment form shows the subjects you are enrolled in, the number of units of each subject, and the school fees you need to pay in full or installment.
  6. Proceed to the Cashier to make to your full or down payment in cash. Full or partial payment shall be within the scheduled enrollment period.
    NOTE: Delayed payment may result in the forfeiture of your admission and the slot assigned to you may be given to other applicants on the waitlist.
  7. Requirements:
    • Medical certificate—from CLDHEI-accredited clinics—stating that the student is fit to study. Please bring original copies of laboratory test results for verification purposes.
    • Photocopy of PSA birth certificate
    • Crtificate of good moral character
    • Marriage certificate, if married
    • 2 pcs 2" x 2" colored iD picture
    • 2 pcs 1" x 1" colored ID picture
    • 1 pc long plastic envelope
Step 3 — Payment of Fees
  1. Present the Pre-Assessment form to the Cashier
  2. Pay the full amount or (if installment) the required down payment. Payments may be in cash or through direct bank deposit, online banking, GCash, or Cebuana Lhuillier. Keep your transaction records for reference. Full or partial payment should be made within the enrollment period.
  3. The Cashier shall issue the original copy of the official receipt (OR) for the amount you paid in cash or (if payment is not in cash) once your payment is verified.
  4. The Cashier shall issue a Certificate of Registration form showing the subjects you are enrolled in and the the payment schedule of tuition and other school fees.
  5. Keep your copy of the OR and Certificate of Registration form for future reference
  6. You are now officially enrolledin CLDHEI!

C U R R E N T / O L D   S T U D E N T S
Step 1 — Curriculum Monitoring & Advising
  1. Update your Student Program Curriculum (the blue form) and have it signed by one of your parents or your guardian. Forms are available at the Admissions & Registration Office
  2. Present the updated blue form to the program dean, head or authorized representative for curriculum monitoring and advising.
  3. After the curriculum monitoring and advising session, proceed to the Admissions & Registration office to continue with your enrollment.
  4. Requirements:
    • Student Program Curriculum signed by parent or guardian
    • Copy of final grades of the just ended semester
Step 2 — Registration & Assessment
  1. Present Clearance form and the signed Student Program Curriculum (blue form) with attached copies of the final grades to the Admissions & Registration Office.
  2. The Admissions & Registration Office will issue a copy of your Pre-Assessment form. The form shows the subjects you are going to enroll in, the number of units for each sbbject, and the initial estimate of the amount of tuition and other school fees that you will be paying.
  3. If the pre-assessed information is acceptable to you proceed to the Cashier within the same day or in any day within the scheduled enrollment period.
  4. Requirements:
    • Clearance form completed for the just ended semester
    • Student Program Curriculum (blue form) signed by the program dean/head
    • Copy of final grades from just ended semester

  1. Present the Pre-Assessment form to the Cashier
