REQUEST FOR DOCUMENTS

Procedure - Request for Documents

We shall only accept Request for Credentials online via email. Refer to the procedure below for your reference. We request your patience and understanding that the processing time may take longer than the usual because we are operating with skeleton force only to ensure the safety of our employees.

Step 1 – Email Request Form

– Email accomplished Request for Documents Form Registrar Form 0009-2018 with 1 valid ID to the following:

2. norabquiaoit@cldhei.edu.ph
Indicate Email Subject: Name of Student – REQUEST FOR DOCUMENT (If TOR, Copy of Grades etc.) 1 Document 1 Email

Step 2 – Pay thru Metrobank or Instapay

– Pay upon receipt of advice via email from Accounting Department of the payment order and instruction for payment
– Payment is thru Metrobank deposit online or over-the-counter and or via Instapay fund transfer

Step 3 – Email Proof of Payment

– Email the proof of payment to the Accounting Department and an Official Receipt (OR) shall be sent once payment has been verified.
– Payment details will then be forwarded by Accounting to the Registrar’s office for processing of the documents.
– Wait for advice via email from Registrar’s office for the schedule of pick up of documents

Step 4 – Pick Up Documents Requested

– Pick up documents as scheduled and follow safety protocols
 
1. No Face Mask no entry
 2. Social Distancing
 3. Answer Health Checklist
– Bring authorization letter and 2 valid ID’s for those who shall claim through a representative
